Description

• Conduct assessments for patients in inpatient and outpatient environments and develop tailored treatment plans
• Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Provide education to patients and families regarding therapeutic objectives, discharge planning, and recommended home exercises
• Maintain comprehensive documentation of evaluations, treatments, and progress in compliance with facility policies and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
