- Conduct assessments for patients in inpatient and outpatient environments and develop tailored treatment plans
- Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
- Provide education to patients and families regarding therapeutic objectives, discharge planning, and recommended home exercises
- Maintain comprehensive documentation of evaluations, treatments, and progress in compliance with facility policies and regulatory standards
- Engage in care team rounds and interdisciplinary meetings to ensure cohesive care delivery
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Additional certifications (e.g., neurological, geriatric) are beneficial but not mandatory
- Previous experience is highly desirable
- Ability to provide care for patients with intricate medical and mobility challenges
- Solid communication and teamwork skills
- Comfortable utilizing digital record-keeping and clinical documentation systems
