Registered Nurse - Case Manager

Role Case Management Nurse responsible for assessing, developing, implementing, and evaluating discharge planning needs for patients in collaboration with an interdisciplinary team. Responsibilities Ensures compliance with discharge planning standards, identifies barriers to length of stay, assesses high-risk patients for readmission, and collaborates on discharge plans. May also handle utilization management, ED case management, admission processes, and liaise with post-acute providers. Applies evidence-based practices, adheres to professional nursing standards, and maintains patient safety. Qualifications Graduate of an accredited nursing program, with 3+ years of acute care experience. Requires RN licensure in relevant states, excellent communication, attention to detail, and self-motivation. Physical ability to perform typical nursing duties is necessary. Skills Strong communication, critical thinking, and collaboration skills. Knowledge of healthcare protocols and patient care processes.
