About the Role

We are seeking a full-time Licensed Practical Nurse (LPN) to provide clinical services to all clients receiving ACT team services, which includes medication monitoring and documentation; delivering psychopharmacologic and medical services within the scope of practice of the individual's license; service coordination, including working with the ACT team to monitor each individual's clinical status and response to treatment; crisis evaluation and intervention; symptom evaluation and management; individual counseling and psychotherapy; substance abuse treatment; work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services; support services or direct assistance to ensure that clients obtain the basic necessities of daily life; and education, support, and consultation to individuals' families and other major supports.

• Monitor and oversee all medical/clinical aspects of services delivered.
• Provide medication monitoring and documentation.
• Deliver psychopharmacological and medical services within your scope of practice.
• Collaborate with ACT team members to monitor clinical status and response to treatment.
• Conduct crisis assessments, symptom management, and intervention planning.
• Provide individual counseling, substance abuse treatment, and psycho-educational support.
• Assist in creating transition plans when clients move in and out of services.

Compensation

Competitive hourly pay aligned with your role, experience, and contribution, with opportunities for growth over time.

• Compensation: $25.02 - $32.13

Placement within the range is based on licensure level, education, and experience.

Responsibilities

• Maintain accurate and timely clinical documentation
• Collaborate with care teams as needed
• Maintain compliance with all clinical and regulatory standards
• Participate in supervision (if applicable based on licensure level)

Qualifications

• Education/Licensure : Active LPN license in North Carolina.
• Experience : At least 1 year of experience working with the identified population.
