Schedule Job Description We are seeking a dedicated Home Health Speech Language Pathologist (SLP) to provide exceptional care to adult and geriatric patients in their homes. The ideal candidate will possess a valid Oregon state license and have at least one year of skilled home health experience. Responsibilities Conduct evaluations and develop treatment plans for patients with speech and language disorders. Implement therapeutic interventions and strategies to improve patient outcomes. Document patient progress using charting systems, including EPIC Remote Client and Rover/Oasis. Communicate effectively with patients, families, and interdisciplinary team members. Provide patient and family education on effective communication strategies. Required Experience / Certifications / Licensure 1 year of skilled home health experience required. Oregon state licensure as a Speech Language Pathologist is mandatory. BCLS certification required. Experience with OASIS is highly preferred; please specify in application.
