Community Health Navigator & Care Coordinator

Saint Francis Healthcare System is seeking a Community Health Worker to coordinate outreach, education, and care coordination services to engage patients in preventive and ongoing healthcare. The role supports treatment adherence, helps remove barriers to achieving care plan goals, and connects patients with community resources.
