Job Summary:
We are seeking dependable, community-focused Case Managers to join our network as  Independent Contractors (1099) . This is a  non-clinical role focused on field-based support. You will not be diagnosing clients or providing therapy. Instead, you will work autonomously in the community, helping clients build daily living skills, locate resources, and stay on track with their care plans.While this role offers a  flexible schedule where you can manage your daily appointments and calendar, you must be willing to commit to 40 hours of service per week .

Key Responsibilities (What you will do):

• Skill Building: Help clients practice daily life skills like budgeting, organizing, and managing stress.
• Resource Connection: Guide clients to community resources like housing, food pantries, and employment offices.
• Appointment Support: Remind and assist clients with attending medical or community appointments.
• Progress Tracking: Write simple, objective daily notes about the client's progress to submit to the host clinical team.

Contractor Requirements (What you must provide):
As an independent contractor, you are responsible for maintaining your own professional business setup. You must possess or obtain:

• Ability to commit to 40 hours a week of client service and administrative tracking.
• A registered  EIN (Employer Identification Number)or be prepared to file via SSN.
• Professional Liability Insurance (Malpractice insurance for case managers).
• A reliable vehicle, valid driver's license, and active auto insurance for community travel.
• An active individual  National Provider Identifier (NPI)number.
• An active individual  Ohio Medicaid Provider ID linked via the PNM portal.

Qualifications & Credentials:

• High School Diploma, Associate's, or Bachelor's degree.
• Must meet Ohio requirements for a  Qualified Mental Health Specialist (QMHS)or  Qualified Behavioral Health Specialist (QBHS)(proven via past certificates or completion of verified competency modules).
• Preferred: 3+ years of documented experience working in community behavioral health (allows for higher contract billing tiers).
