Licensed Practical Nurse - Primary Care

LPN Care CoordinatorSupervised by a Registered Nurse or Physician, the LPN guides our patients through a seamless process to ensure that their care is of the highest quality and delivered in the most efficient manner. Directing the patients through the appointment, provisions of nursing services during care, and phone triage. Collaborates in care planning discussions, based on RN, MD or other Providers -determine urgency of care needs, triage of emergent care situations to the appropriate provider, and scheduling of acute care appointments to maximize patient care opportunities and unit efficiency. Maintains the flow of the Provider schedule during patient care sessions minimizing patient wait time whenever possible.
