Case Manager - Addiction Recovery & Discharge Planning

AVENUES RECOVERY CENTER

Avenues Recovery Center in Fort Wayne is seeking dedicated case managers to advocate for clients and facilitate their treatment journey. You will initiate case management, address treatment barriers, and arrange post-discharge support, ensuring a seamless recovery process. We are looking for candidates with a high school diploma or GED, and preferred experience in the treatment industry.
