RN Case Manager - Home Health

Role & Responsibilities Provide personalized home-based care to patients recovering from illnesses or managing chronic conditions. Collaborate with physicians and oversee LPNs, CNAs, and aides to deliver comprehensive care. Conduct patient assessments, document progress, and communicate changes to healthcare providers. Educate patients and their families on care plans, medications, therapies, and safety measures. Ensure care goals are achieved and coordinate patient discharge processes.
