Job Description
Job Title: Home Health RN

Description:
Utilizes the nursing process to assess patient/caregiver needs in order to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient.
Ensures that the patient/caregivers are included in the care plan process.
Serves as Case Manager for assigned patients and actively manages case loads and visit times.
Completes OASIS documentation in accordance with Agency policies and procedures.
Demonstrates knowledge of OASIS documentation requirements and ensures timely completion.
Implements medical plan of care, nursing interventions, and procedures necessary for patient care.
Stays current with a variety of High-Tech skills, including IV use and maintenance and complex wound care.
Maintains infection control standards throughout the plan of care.
Works collaboratively with LPNs to ensure that the plan of care is followed and patient needs are met.
Reviews documentation and acts on recommendations in accordance with policies and procedures.
Evaluates aspects of the implemented plan of care to meet patient/caregiver needs.
Maintains accurate documentation of nursing care to ensure ongoing quality of care.
Follows agency protocol with documentation processes.
Performs patient and caregiver education to facilitate informed decisions regarding care.
Identifies interdisciplinary needs of the patient and communicates this information timely.
Completes various forms in a timely manner to facilitate patient care.
Carries out visits efficiently and communicates scheduling information promptly.
Assesses patients' needs for services and coordinates plans of care with patients and caregivers.
Transmits patient incident reports to the appropriate specialists in a timely manner.
Coordinates safe patient discharges or transfers and ensures all paperwork is processed properly.
Facilitates communication and coordination of patient care with healthcare team members.
Participates in monthly peer review audits and provides and accepts constructive feedback.
Proactively schedules and facilitates case conferences to maximize effective communication with team members.
Advocates for patient care issues to ensure quality and type of care is sensitive to specific patient needs.
