About the Role:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our night shift team. As an LPN with Green Palms Health & Rehab at Crawford, you will play a vital role in delivering high-quality care to our residents. Your responsibilities will include:

• Assisting with the delivery of patient care, including medication administration, wound care, and vital sign monitoring

• Providing emotional support and social interaction to our residents

• Collaborating with other healthcare professionals to develop and implement individualized care plans

• Maintaining accurate and detailed records of patient care and progress

Responsibilities:

• Provide direct patient care, including administering medications, treatments, and procedures as ordered by physicians

• Conduct vital sign checks and report any changes or concerns to the nursing staff

• Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Maintain a clean and safe environment for our residents and staff

• Participate in resident care conferences and family meetings

• Follow established policies and procedures for infection control, fire safety, and emergency procedures
