What Makes Home Care Different?
As an LPN on our Paul Revere team, you will:
- Deliver 1:1 skilled nursing care
- Work collaboratively with RN Case Managers
- Practice at the top of your license
- Provide complex clinical treatments
- Support patients recovering from surgery, illness, or chronic disease exacerbations
You won't "manage" patients you'll focus on delivering high-quality, hands-on clinical care and education that prevents hospital readmissions and improves lives. Primary coverage will be in: Dorchester, Mission Hill, Roxbury, Jamaica Plain.
Schedule & Workload
Full-time (40 hours) preferred, 32-hour and 36-hour options available. MondayFriday schedule, average of 67 visits per day. Dedicated time for documentation, care coordination, and chart review. No hospital shift chaos. No overnight rotations. No weekends. Just meaningful, focused patient care!
Qualifications
LPN professional license in Massachusetts
Minimum 1-year previous nursing in acute care or sub-acute care setting required
Home care experience preferred
Effective written, verbal and electronic communications skills
Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred
Strong interpersonal skills with ability to work with varied population
Ability to work as a member of an interdisciplinary team
Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
Travel required in local geographic area
Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population.
