Work Environment

This role is based in our state-of-the-art outpatient office in Tracy, California. We have a structured schedule from Monday to Friday, 9 AM to 5 PM, providing a balanced work-life dynamic. Key Responsibilities

As a Physical Therapist at Vital Core Physical Therapy, you will:

• Conduct thorough evaluations of patients, identifying their unique needs and developing tailored treatment plans that address their specific goals.
• Utilize evidence-based treatment approaches, including therapeutic exercises, manual therapy, neuromuscular re-education, and various modalities to enhance recovery.
• Provide patient education on injury prevention strategies, posture adjustments, ergonomic techniques, and effective self-management skills.
• Continuously monitor and document patient progress, making necessary adjustments to treatment plans based on ongoing assessments and outcomes, ensuring comprehensive Electronic Medical Record (EMR) management.
• Collaborate effectively with an interdisciplinary team, including referring physicians, orthopedic surgeons, and other healthcare professionals, to provide integrated and patient-centered care.
• Engage in regular team meetings and case discussions to share insights and best practices, fostering a culture of continuous learning.
• Mentor and guide Physical Therapy Assistants (PTAs), aides, and student interns when applicable, fostering their professional development.

Patient Population / Client Focus

Our clientele primarily consists of individuals seeking rehabilitation for orthopedic injuries, post-surgical recovery, and chronic pain management. You will work with a diverse range of patients from various backgrounds, including athletes recovering from sports injuries and individuals needing rehabilitation after orthopedic surgeries. Your expertise will play a crucial role in helping these patients regain their mobility and improve their functional capabilities.

Qualifications

To be a strong candidate for this position, you must meet the following requirements:

• Hold a valid physical therapist license in the state of California.
• Possess a Bachelor's, Master's, or Doctorate in Physical Therapy (DPT preferred).
• Demonstrate a minimum of two years of experience in an outpatient orthopedic setting.

Preferred Experience

In addition to the basic qualifications, the ideal candidate will have:
