LICSW | Licensed Independent Clinical Social Worker

Job Summary: Join a multidisciplinary team as a Behavioral Health Therapist providing comprehensive behavioral health services including initial evaluations, individual and group therapy, assessments, treatment planning, and discharge coordination. This role emphasizes evidence-based, time-limited therapy addressing psychological, psychosocial, environmental, age-specific, and cultural factors to enhance patients' social, emotional, and physical well-being.

Essential Functions:

• Conduct screenings and assessments for behavioral health and substance use disorders, facilitating patient engagement and follow-up care.
• Deliver brief and extended behavioral interventions using evidence-based methods such as behavioral activation, problem-solving treatment, motivational interviewing, and cognitive behavioral therapy.
• Support the mental and physical health of patients within an assigned caseload, maintaining professional relationships with patients and care team members.
• Collaborate closely with medical providers and psychiatric consultants to develop integrated health and behavioral health-substance use disorder care systems aimed at reducing emergency and crisis utilization.
• Work with chronically ill or high-risk patients, their families, caregivers, and community resources to coordinate care effectively.
• Monitor patient progress through medical record audits, intervening as needed to ensure adherence to care plans and maintaining communication with care partners.
• Assist in developing programs that promote patient self-management and shared decision-making, providing education on treatments and procedures.
• Educate patients about common behavioral health and substance use disorders and available treatment options.
• Establish and maintain therapeutic relationships to promote adherence to individualized care plans developed collaboratively with patients, providers, and families.

Education and Experience Requirements:

• Must hold specialized training and/or clinical licensure to provide evidence-based psychosocial interventions such as problem-solving treatment and cognitive behavioral therapy.
• Licensure options include Licensed Independent Clinical Social Worker or Licensed Psychologist.
• Demonstrated communication, organizational, and counseling skills.
• Experience with health information technology systems.
• Basic understanding of psychopharmacology related to common behavioral health disorders.
• Knowledge of differential diagnoses for behavioral health and substance use disorders.
• Familiarity with evidence-based psychosocial treatments and behavioral interventions (e.g., CBT, motivational interviewing, behavioral activation).
• Ability to collaborate effectively with healthcare providers and team members.
• Understanding of Medicare, Medicaid, third-party payers, and BH-SUD coding.

Preferred Qualifications:

• Experience using Epic electronic health record system.
