Registered Nurse RN Case Manager Hospice - Rockland & Orange County NY

Job Description SORRY , THIS IS NOT A REMOTE POSITION. This is a patient-facing position, our patients are located throughout Rockland and Orange County, NY. We are an employer that believes in work-life balance and does the work to provide it, not just talk about it. To be an RN - Case Manager you need to be: Dedicated to the philosophy of hospice care. Believe that excellent patient care is the only acceptable standard. A Registered RN, licensed to practice nursing in New York State. Comfortable providing community-based care. Some experience caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy of care. One to two years of recent bedside nursing experience with good clinical skills. Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs. Ability to relate to a diverse population and workforce in a personable and effective manner. Love to work on a team of dedicated professionals from multiple disciplines. Must possess a car, valid driver's license, and current insurance and arrange other means of transportation if your personal car is unavailable. Salary for this position is $74k-$105k annually and is commensurate with education and applicable experience.
