The Primary Care Physician (PCP) in Our Organization Demonstrates Accountability for outcomes: accounting for outcomes, strong clinical care and cost‑effectiveness for up to 450 patients; building trusting relationships and helping patients change behaviors. Coaching for health: acting as a health coach rather than a sickness consultant; setting short‑ and long‑term health goals, partnering with patients and frequently following up. Simplifying for action: simplifying and prioritizing appropriately so that behavior change is actionable for patients and for the care team. We are an outcomes‑focused, value‑based organization. Metrics for PCPs include patient admissions per thousand, appointment slot usage (18‑21 per day), CGCAHPS, clinical gap closures, and medical cost measures. PCPs set and work toward these goals with leadership and team members. Culture is essential; PCPs champion a positive culture of love, accountability and passion. Essential Job Duties / Responsibilities Provide independent care for acute and chronic illnesses in older adults. Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved. Engage with the hospitalist whenever a PCP’s patient is hospitalized. Perform assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for patients with acute and complex chronic needs. Lead a care team of medical assistant, care facilitator and care coordinator for office‑visiting patients. Engage with transitional care teams for patients unable to visit the office (hospital, SNF, LTC, homebound). Play an active role in center management, cover for other providers, and recruit patients and additional providers as needed. Perform other duties as assigned by management. Knowledge, Skills & Abilities Availability and Accessibility for patients – open for walk‑in visits and prompt phone/message response. Service Orientation – deliver care that a physician would offer a family member. Evidence‑Based Medicine – stay current with medical, nutritional and lifestyle interventions. Physician Leadership – continuously develop leadership skills for benefit of patients, team, center and company. Quality – work toward high‑quality outcomes and improve practice systems. Influence – competently and compassionately influence patients, teams and self. Self‑Care – maintain physical, mental, social and spiritual well‑being for sustainable practice. Proficiency in Microsoft Office Suite and related software. Willingness to travel locally, nationally for initial training, and regionally/nationally for recruiting or training as needed. Fluency in English (verbal and written); fluency in other languages may be required in some centers. Use independent judgment. Pay Range $221,141 – $315,915 annual salary. Final compensation depends on experience, education, location and other factors. Bonus and commission opportunities may apply.
