Must be licensed in the State of Missouri as a Registered Nurse, have excellent communication and computer skills as well as reliable transportation. Job Responsibilities Assist the men with scheduling and coordination of health care specialists Accompany Jesuits to physician or specialist appointments as needed Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership Manage admissions and readmission, including communication of condition, treatments, and medications Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty Assume full responsibility for assigned cases Complete documentation, charting, and planning in accordance with prescribed procedures Requirements Current Registered Nurse (RN) license in the State of Missouri CPR certification required Minimum of one (1) year experience in an adult medical/surgical unit Six (6) months of home care experience preferred Affirmative Action/Equal Opportunity Employer/Females/Minorities/Disabled/Veterans Delmar Gardens is a family-owned, St. Louis-based company that has been serving the community for 60 years.
