Hospice Social Worker (BSW or MSW)Full-Time | Monday–Friday | 8:00 AM – 5:00 PM Serving the Martinsville, VA Service AreaAt Mountain Valley, what we do is personal. This role is part of an interdisciplinary team focused on providing comfort, dignity, and support wherever patients call home.What You'll DoProvide emotional and psychosocial support to patients and familiesComplete assessments and contribute to individualized care planningAssist with advance care planning and end-of-life decision supportConnect patients and families with community resourcesSupport with grief, coping, and caregiver stressCollaborate closely with nurses, aides, chaplains, and physiciansParticipate in IDG meetings and team-based care planningWhat We're Looking ForBachelor's or Master's degree in Social Work (BSW or MSW)Current social work license (or eligible) in VirginiaAt least 1 year of experience in healthcare, hospice, or a related human services settingStrong communication and relationship-building skillsAbility to work independently while staying connected to a teamCompassion, presence, and the ability to meet people where they areWhy This Role MattersThis role gives you the opportunity to do meaningful, one-on-one work with patients and families at the end of life.
