Job Description Summary The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry‑level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Responsibilities (listed in order of importance and/or time spent) Determine the suitability of individual patients for home care. Collect and organize all patient‑specific information needed to determine suitability, assess medical condition and prescribed medication therapy, and define outcome goals. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients, making modifications in medication therapy or monitoring parameters as appropriate. Collect and organize information to prevent, detect and resolve medication‑related problems and identify appropriate monitoring parameters. Complete nutritional assessment and make recommendations. Modify the pharmacy care plans based on evaluation of monitoring data and other pertinent patient‑specific information. Apply pharmacokinetic principles and formulas and assess patient laboratory values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques. Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings. Communicate pertinent patient information systematically. Prepare and dispense medications following Option Care policies, calibrate equipment, prepare appropriate concentrations, inspect final products, comply with state/federal laws, and adhere to safety and quality assurance practices. Manage the use, maintenance and troubleshooting of medication administration equipment and related equipment used by home care patients. Provide programming, maintenance advice and participate in incident reporting. Complete QAR documents and monitor inventory discrepancies for schedule II narcotics and other controlled substances. Provide concise, timely responses to drug‑information requests from health care providers and patients, formulary analysis and CEU opportunities to promote professional competency. Manage investigational drug products according to regulations and Option Care policies. Utilize pharmacy support personnel effectively, prioritize workload, and ensure accuracy of their work. Comply with accreditation, legal, regulatory and safety requirements in all activities. Maintain confidentiality of patient and proprietary information, observing legal and ethical safeguards. Supervisory Responsibilities Does this position have supervisory responsibilities? No X Yes Basic Education And/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. One year of experience in sterile IV admixture procedures or knowledge of non‑sterile compounding, immunizations and medication therapy management, completion of an ASHP accredited Pharmacy Residency Program, an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience approved by the Vice President (VP) of Pharmacy and/or Senior Vice President (SVP) of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction. Basic level skill in Microsoft Word (opening documents, formatting, editing, and printing). Basic level skill in Microsoft Excel (opening workbooks, formatting, using formulas, and printing). Basic skill level in Microsoft PowerPoint (creating, editing, and presenting slides). Travel Requirements Willing to travel up to 5 % of the time for business purposes (within state). Preferred Qualifications & Interests (PQIs) Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital or other clean room setting. Management Requirements At least 1 year of experience in indirect management of team members, including assisting in development, training and assignment of work/projects. Salary Pay Range: $97,113.10 – $161,862.67. Salary determined by education, experience, knowledge, skills and abilities, internal equity and market data.
