This is an on-site Telephonic Nurse role where Licensed Practical Nurses support patients through phone-based care coordination and medical management instead of bedside work. The role focuses on communicating with patients, providers, and plan sponsors to ensure care needs are met while promoting efficient, cost-effective outcomes.
