This role is designed specifically for someone with lived experience in addiction recovery, capable of providing support through a peer-based, recovery-oriented approach. Supportive and positive work culture Opportunities for career advancement Requirements HS Diploma or degree in Social work or related field Experience serving as a sponsor or working a 12‑step program is strongly preferred Active involvement in a structured recovery program Strong understanding of the 12 steps and sponsorship principles Flexible schedule to work off hours Strong organizational and communication skills Ability to collaborate with a multidisciplinary team and external agencies Knowledge of community resources and support services Commitment to promoting a culture of diversity, equity, and inclusion Responsibilities Conduct comprehensive assessments to identify clients' needs and develop individualized care plans. Coordinate and advocate for the delivery of a range of services to meet clients' mental health and substance abuse treatment goals. Collaborate with healthcare professionals, social services, and external agencies to ensure continuity of care. Provide support and guidance to clients in accessing community resources and support services. Maintain accurate and up‑to‑date case records and documentation. Facilitate communication and collaboration among the treatment team to ensure a cohesive and person‑centered approach. Monitor and evaluate clients' progress toward treatment goals.
