RN Case Manager Supervisor

Role Overview Responsible for initial patient review, triage, and assignment to case managers, coordinating multidisciplinary, patient-centered care to ensure quality and cost-effective outcomes. Supervises staff, acts as a resource, and serves as a primary contact for patients, families, and healthcare team members. Manages patient care across all age groups, develops comprehensive care plans, and facilitates discharge planning. Ensures adherence to policies, monitors interventions, and promotes effective communication within the team. Provides education, advocates for patient safety, and participates in team meetings and administrative tasks to support continuous improvement. Qualifications & Skills Bachelor’s Degree in a related discipline; Master’s preferred 3+ years of relevant experience RN licensure in NM or reciprocal agreement; BLS certification required Strong leadership, assessment, collaboration, and communication skills Ability to supervise staff, coordinate resources, and ensure quality patient care Knowledge of patient safety protocols and interdisciplinary team functions Key Responsibilities Perform patient assessment, triage, and needs evaluation Develop and oversee multidisciplinary care plans and discharge strategies Supervise and train staff; manage case loads and coverage Facilitate communication among patients, families, and healthcare providers Ensure compliance with policies, safety standards, and documentation requirements Participate in quality improvement, staff development, and administrative activities #J-18808-Ljbffr
