OACHC is the primary source for training and technical assistance (T/TA) for all 61 of Ohio’s Community Health Centers.
PRIMARY OBJECTIVES: Under the direction and leadership of the Integrated Health Program Manager the Clinical Quality Program Coordinator coordinates and supports the Integrated Health and Quality team with projects, networking calls, and other essential functions.
QUALIFYING CHARACTERISTICS: The ideal Clinical Quality Program Coordinator embodies the Mission, Vision, and Ideals of OACHC. A strong value for cross-functional adaptability, along with the ability to support the Integrated Health and Quality teams in projects, networking, and other areas, is key to success in the Clinical Quality Coordinator role. A successful Clinical Quality Program Coordinator will operate in a manner consistent with OACHC’s Core Ideals (Strategy, Innovation, Collaboration, and Continuous Improvement). Requirements:EDUCATIONAL AND EXPERIENCE REQUIREMENTS: Bachelor's degree in public health, health administration, nursing, OR related combination of education and experience. At least 2 years’ relevant experience is required with demonstrated outcomes-based achievements. Strong preference given to candidates with experience with Community Health Centers or membership associations. Experience with data-driven decision making, program evaluation, and TA delivery are highly desirable.

- Ability to manage project/networking call goals, schedules, and new information, keeping materials up to date.
- Ability to record meeting minutes accurately.
- Participation in strategy, tracking, and analyzing project risks, goals, and outcomes.
- Ability to manage, analyze, and validate project data.
- Ability to participate as part of the Integrated Health and Quality departments for the betterment of the team.
- Demonstrate and use knowledge of quality improvement and practice transformation processes.
MAJOR RESPONSIBILITIES:
- Provide training, coaching, technical assistance, and resources to Community Health Centers related to quality improvement, performance improvement tools, PCMH recognition, and ongoing quality initiatives.
- Provide technical assistance related to data reporting, data displays, data cleaning, data interpretation, data collection, program evaluation, and reporting to support quality improvement and grant initiatives.
- Coordinate and facilitate learning networks and collaborative meetings, including meeting logistics, agendas, communications, recording, minutes, attendance tracking, satisfaction surveys, and follow-up activities.
- Coordinate, support, and implement projects focused on integrated health, chronic disease, cancer screening and prevention, and other related quality improvement priorities.
- Collect, assemble, and distribute data displays to member health centers, including but not limited to, quarterly data sharing reports and data validation reviews.
- Serve as the primary point of contact for emergency preparedness activities, including coordinating related initiatives, communicating resources and updates to CHCs, participating in state and national emergency preparedness meetings and workgroups, and sharing relevant information and best practices with internal and external stakeholders.
- Support development of required grant and project progress reports; maintain documentation; monitor project activities and outcomes to support successful completion of initiatives.
- Promote and share best practices, successful strategies, resources, and emerging trends among CHCs through meetings, webinars, communications, and other learning opportunities.
- Develop and maintain effective relationships with internal and external partners, state agencies, coalitions, and other key stakeholders. Represent the organization and present concepts, project updates, and relevant information as needed.
- Plan, coordinate, and facilitate internal and external meetings, trainings, webinars, conference calls, and other educational activities. Coordinate with Integrated Health and Quality teams to support training and technical assistance planning, implementation, delivery, and reporting.
- Maintain current knowledge of quality improvement methodologies, PCMH standards, QHC regulatory and compliance requirements, including HRSA Program Requirements, FTCA, Risk Management, Emergency Preparedness, as well as emerging healthcare trends, evidence-based resources, and national benchmarks.
- Participate as a member of the Integrated Health and Quality Teams, support ongoing team priorities and projects, maintain relevant portal and communication resources, and coordinate activities and information across internal teams and external participants.
- Perform related responsibilities as assigned by the Integrated Health Program Manager.
