Job Summary / Purpose

• This project supports the urgent expansion of intake and placement operations for families transitioning out of the Row and into the shelter system. In alignment with the Agency's objective of completing all family transitions by January, operations will accommodate up to 60 families per day and operate during extended hours to meet the accelerated timeline.
• The Outreach Specialist plays a key role in ensuring a smooth, safe, and efficient transition process by providing direct support to families throughout their relocation. This position requires strong communication, coordination, and organizational skills while working in a fast-paced, deadline-driven environment.

Key Responsibilities

• Provide direct support to families during their relocation into the shelter system.
• Assess family needs and gather required documentation for intake and placement.
• Identify appropriate shelter placements based on family composition, school enrollment, medical needs, and other relevant considerations.
• Coordinate with internal Agency teams and contracted service providers to facilitate timely and efficient transitions.
• Maintain accurate case records and documentation.
• Assist with case updates and monitor the progress of family transfers.
• Ensure all interactions are conducted with professionalism, empathy, and respect for family dignity.
• Handle sensitive and confidential information with discretion.
• Adapt to changing priorities and support extended operational hours as needed.

Required Qualifications & Skills

• Valid driver's license (Driving required).
• Strong verbal and written communication skills.
• Excellent interpersonal and customer service skills.
• Ability to collaborate effectively with diverse teams and stakeholders.
• Strong organizational and time management abilities.
• Ability to manage sensitive information with confidentiality and professionalism.

Preferred Qualifications

• Experience in case management, social services, community outreach, shelter operations, or human services.
• Experience working with vulnerable populations or families in transition.
• Familiarity with intake, placement, or housing support services.

Work Environment

• Fast-paced, high-volume operational setting.
• Extended work hours may be required to support project timelines.
• Frequent coordination with internal teams, external partners, and client families.
• Local travel required; a valid driver's license is mandatory.
