If so, apply today!As a Hospice Aide, you'll contribute to our success in the following ways:Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.Document medication regimens and maintain accurate, up-to-date medication profiles.Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.What is Required?High School Diploma or GED equivalent required1 year of experience as a Hospice Aide or Nursing Assistant is requiredState-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation programAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.Experience in a home health environment is preferredWhat's in it for you?Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance401(K) with employer matchTuition reimbursement for full-time staff and continuing education opportunities for all employees at no costPaid time off, paid holidays, family, and pet bereavementFleet vehicle program (restrictions apply) or mileage reimbursement.* Benefit eligibility can vary by position and shift status.Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
