Join CareMore Health to make a profound impact by leading coordinated care and practicing value-based medicine.Practice InfoCare for medically complex seniors living in skilled nursing facilities and long-term care settings.Patients often face multiple chronic conditions, frequent transitions of care, and significant health challenges.Partner with nurses, care managers, social workers, specialists, facility staff, and pharmacists to create comprehensive care plans.Focus on proactive interventions, quality measures, and long-term outcomes rather than episodic treatment.ResponsibilitiesProvide comprehensive primary care services to ISNP members in SNF and LTC environments.Perform admission assessments, routine visits, and acute evaluations.Develop individualized care plans for medically complex patients.Lead transitions of care following hospitalizations.Support advanced care planning and patient education.Drive quality measures and population health outcomes.CompensationCompensation Range: $211,369 to $317,053The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.RequirementsMD or DO from an accredited medical schoolActive Arizona medical licenseDEA registrationBoard Certified or Board EligibleExperience with geriatrics, post-acute care, SNF, LTC, or complex patient populations preferred
