Primary Duties

Designs and executes personalized treatment programs based on Physical Therapy principles and methods. Works closely with an interdisciplinary treatment team. Reports to the Clinical Coordinator of Rehab Services.

Key Tasks

Evaluation & Treatment Planning for Patients

• Assesses requests for Physical Therapy services
• Conducts patient interviews and evaluates their physical status and requirements
• Creates customized exercise and treatment plans

Therapy Techniques

• Determines suitable equipment usage, application, and body positioning
• Administers treatments such as exercise, gait training, and various therapies
• Educates patients on regimen components and proper use of aid devices
• Adjusts treatment plans based on patient progress
• Collaborates with physicians on treatment options as needed

Documentation & Coordination

• Completes necessary documentation, including evaluations and progress notes
• Communicates effectively with team members to ensure coordinated care

Team Support & Guiding

• Provides guidance to supporting staff involved in patient care
• Takes on responsibilities of Senior Therapists as needed

Quality, Safety & Compliance

• Adheres to departmental policies, procedures, and quality standards
• Maintains a safe environment for all parties involved
• Follows all safety protocols and procedures
• Uses equipment properly and ensures it is in working order
• Adheres to precautionary measures and safety protocols
• Reports unsafe conditions and contributes to safety protocols

Operational Assistance

• Aids in the care and upkeep of department equipment and supplies
• Participates in required meetings and committees
• Assists with patient transportation as necessary

Professional Growth

• Engages in continuous education and professional development opportunities
• Completes mandatory annual training requirements

Focus on Patient Care

• Delivers age-appropriate care for patients across all age groups
• Provides attentive care focused on physical and emotional patient needs
• Communicates effectively to ensure a positive patient experience

Requirements for Education

• Must be licensed or licensure-eligible for Physical Therapy practice in New York State at time of hiring

Work Experience

• Prior experience in a hospital setting preferred

Certifications

• Basic Life Support (BLS) certification required within 90 days of being hired

Physical Requirements

This role entails physical tasks, including lifting and maneuvering. Must be able to:

• Lift at least 30 pounds independently
• Lift up to 150 pounds with assistance
