Practice Highlights:

• Predominately outpatient clinical role with meaningful medical director leadership opportunities

• No call, no inpatient responsibilities and no home care
\n
• Small, intentionally designed patient panel that supports high-quality geriatric care

• Opportunity to help shape the geriatric care model across primary care and consult services

• Operational dyad partnership to advance clinical priorities, care pathways and team development

• 80% clinical (PCP + geri consult clinic) 20% leadership, supported by an operational dyad

Role & Scope of Practice:

• Deliver outpatient geriatric primary care and consult services
\n
• Partner in a dyad structure to translate clinical priorities into operations
\n
• Expand the team by adding 2 APPs with leadership support
\n
• Collaborate with palliative care and specialty partners to strengthen care pathways
\n
