Job DescriptionJob Overview\nEbenezer is looking for aRegistered Nurse - RN whom would provide personal cares to improve and/or maintain the assited living residents’ physical and emotional well-being for our seniors at Meadowwoods Senior Living in our beautiful Bloomington, MN location\n\nNursing schedule includes;\n\n 72 hours in two weeks, full time\n Day shift\n One weekend per month\n Full benefits included RN student loan repayment option\n\nAs part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is in a residential setting by woodlands, wetlands, and the Minnesota River Bluffs. Our 113 unit assisted, independent and memory care buildings are located in East Bloomington, MN near public transportation. Our staff encourages participation in the many regularly scheduled activities and lifelong learning programs. Managed by Ebenezer we believe their mission to heal, discover, and educate for longer healthier, more meaningful lives.\nResponsibilities\n\n Maintains current assessment and documentation of resident health status:\n Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements.\n Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement.\n Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually.\n Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.\n Maintains clinical charts and progress notes using computerized documentation system.\n Communicates changes in resident conditions to medical and other relevant individuals and perform the appropriate follow up to health needs\n Coordinates resident health services:\n Facilitates coordination of services with internal and external providers.\n Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.\n Works with resident and/or representative(s) to assure delivery of appropriate services and funding.\n Manages and oversees medication and treatment administration.\n Assures medications and supplies are ordered in a timely manner.\n Assures medication and treatment practitioner orders are renewed at least annually and as necessary when there is change in medication and/or treatment. Minnesota Assisted Living regulations will be followed in obtaining and implementing practitioner orders.\n Assures medi boxes, bubble packs, and/or syringes are filled per medication orders/changes for medication administration by designated staff.\n Assures that all medication administration procedures are correctly followed. Takes appropriate action with staff regarding any problems or issues.\n Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures.\n Monitors work and performance of licensed and unlicensed staff.\n Meets with staff to coordinate care and address care issues.\n\nRequired Qualifications\n\n Associates Degree in Nursing\n less than 1 year nursing experience\n MN Registered Nurse (RN) License\n Basic Life Support (American Heart Assoc or Red Cross)\n\nPreferred Qualifications\n\n Bachelor of Science Nursing\n 1 year nursing experience\n Prior supervisory experience\n Prior experience in home care or assisted living environment\nQualifications\n$37.67- $54.42 HourlyBenefit Overview\nEbenezer offers a generous benefits package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
