Create and implement individualized treatment plans aligned with resident needs and facility policies. Provide direct therapy and document progress, adjusting goals as appropriate. Collaborate with nursing, rehabilitation, medical, and therapy teams to support safe, efficient resident care and discharge planning. Educate residents and families on communication strategies and dysphagia management; ensure patient safety and adherence to HIPAA standards.
