POSITION SUMMARY
The Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. The CHW functions under the direct supervision of the Health Education Manager to provide direct intervention services and education to clients/participants/community members. The CHW strives to exemplify the mission, vision, and values of Healthways Alliance and the Poor Handmaids of Jesus Christ (PHJC).PRINCIPAL RESPONSIBILITIES AND DUTIES
Facilitate regularly scheduled program sessions using approved curriculum and program materials. Prepare for all classes, including participant materials, incentives, healthy meals, etc. Complete all necessary documentation for each class, such as attendance, screenings, and any other required paperwork.. Conduct outreach aimed at reaching community members to assess their health risks and needs at a variety of locations. Enroll interested individuals in appropriate programs. Provide testing for blood pressure, weight/BMI, and A1C at an initial screening, when possible. Provide post-program screening at predetermined intervals, as specified by the program. (Training for the CHW will be provided.) Develop and regularly update a list of community resources to refer participants to the appropriate agencies and community resources. Identify and provide resources and referral information to those who request them. Follow up with people who have been referred to for services to ensure their needs are addressed. Administer behavior surveys on healthy eating, physical activity, rest, or other health topics to clients and those involved in outreach activities to assess their health knowledge. Track the progress of program participants and identify areas of success and areas for improvement. Document progress, barriers, referrals, outreach, and related activities in accordance with guidelines. Conduct telephone and face-to-face interviews and assessments. Provide one-on-one coaching and follow-up with program participants, encouraging the development of personalized plans and goals that address barriers, using motivational interviewing, counseling, and referrals to support participants in developing and adhering to a healthy lifestyle. Attend and actively participate in community events, such as vendor, back-to-school, and health fairs, to promote programs and services, conduct health screenings, and provide public health information to attendees. Consistently update information in the client database. Record, retrieve, and analyze data as needed to monitor participant and program results and make appropriate referrals and reporting. Assist the Health Education Manager in completing the required grant reports in a timely manner. Complete and maintain monthly reports for information and assistance services. Submit reports to the appropriate agencies in a timely manner. Communicate requirements related to documentation and reporting. Maintain strict confidentiality of information related to services and participants. Maintain official records, communications, correspondence, and files. Review waiting lists and logs to update information and follow up for outcomes. Inform the Health Education Manager of program needs such as supplies and other materials. Support fundraising efforts and other related events to expand organizational recognition in the community and develop strategic partnerships. Participate in ongoing personal and professional development to stay abreast of health risks, including training and conferences as directed. Continually strive to exemplify the mission and values of our sponsors and respect the confidentiality of the organization. Provide a warm introduction to the program for those who inquire. Perform other duties as assigned to meet the vision, philosophy, mission, and values of Healthways Alliance and the Poor Handmaids of Jesus Christ.MINIMUM QUALIFICATIONS
The above level of knowledge, skills, and abilities is normally acquired through completion of an associate degree, certification program, or equivalent level of knowledge obtained through a combination of education and experience. Ability and willingness to work some evenings and weekends when required. Certified Community Health Worker Training (within 6 months of hire / Provided by Healthways Alliance). Type at least 40 wpm. Valid Driver’s License, reliable personal vehicle for driving, car insurance, and good driving record. Driving is required.REQUIRED SKILLS
Knowledge of Outlook and Microsoft Office programs including Word, Excel, and PowerPoint. Ability to work with sensitive information. and maintain confidentiality. Ability to work independently and in a team environment. Ability to manage multiple projects with attention to detail. Initiative, the ability to work independently to plan, organize and set priorities. Exceptional judgment, discretion, and ability to work under pressure and adapt to change. Excellent communication (verbal and written) as well as excellent listening skills. Effective oral and written communication and presentation skills.MACHINES, TOOLS, EQUIPMENT, AND WORK AIDS
