Northwestern Medical Center in Vermont is seeking an Outreach Specialist / Health Coach to join the Population Health Department. This full-time role (80 hours bi-weekly, 8 am–4:30 pm) focuses on chronic disease prevention and patient self-management programs, with a local outreach approach and collaboration with care teams. You will conduct one-on-one coaching, refer patients to MHVT programs, engage with community partners, and support health promotion efforts across Franklin/Grand #J-18808-Ljbffr
