Hearts With A Mission has a great opportunity for a full-time (40 - hours per week) Long-Term Rental Assistance (LTRA) Case Manager in Jackson County.
The Long-Term Rent Assistance (LTRA) program provides rent subsidy to eligible households to increase
housing access and long-term housing stability.

POSITION SUMMARY
The Hearts With A Mission Long-Term Rental Assistance Case Manager (LTRA Case Manager) works as part of an inter-disciplinary service team with participating clients. This position is responsible for assessing client needs, developing individualized service plans, coordinating services and supports, and monitoring client progress toward identified goals. The LTRA Case Manager ensures that accurate and complete information is collected, documented, and maintained to support effective case management and timely delivery of appropriate services. Through a detailed understanding of client needs, available community resources, and program requirements, along with compassionate interaction with clients, families, care teams, and community partners, this position will serve as a link between clients and the services and supports identified through their individualized service plans. Building positive, supportive relationships with clients is central to the effectiveness of this position. The LTRA Case Manager performs assignments with supervision or independently, requiring initiative and judgment in the conduct of work. All work is performed in compliance with organizational guidelines and policies, as well as all applicable laws.
SUPERVISION RECEIVED AND EXERCISED
The Long-Term Rental Assistance Case Manager reports directly to the Services Coordinator. This is not a supervisory position. This position has an introductory period of three (3) months.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Client Services:
- Conduct screening assessments to determine community members’ eligibility for LTRA services.
- Develop individualized service plans in collaboration with clients and families to identify goals, needs, and appropriate supports.
- Provide ongoing case management and support to clients to promote long-term housing stability, engagement, and progress toward identified goals.
- Coordinate services and supports between clients, families, providers, community partners, and other members of the client’s support team.
- Maintain regular and ongoing communication with clients, families, service providers, and other appropriate supports.
- Monitor client progress and follow up to ensure service plan goals and identified needs are being addressed.
- Provide advocacy and support to assist clients in navigating systems, addressing barriers, and accessing appropriate services and resources.
- Collaborate with interdisciplinary teams and service providers to ensure coordination of services and alignment with each client’s service plan.
- Assist clients in identifying and strengthening natural supports and community connections that promote stability and independence.
- Provide information, education, and resources to clients and families to support self-advocacy and informed decision-making.
- Review client needs and service plans to identify barriers, changes in circumstances, and additional supports that may be needed.
- Support clients in establishing connections to educational, vocational, employment, housing, transportation, and other community-based opportunities consistent with their identified goals.
- Promote trust and engagement through consistent communication and supportive relationships with clients and families.
- Consult with Program Managers and appropriate team members regarding client needs, service coordination, barriers, and ongoing case management.
Community:
- Maintain up-to-date knowledge of available community resources, service systems, and programs relevant to client needs.
- Establish and maintain working relationships with landlords, property managers, community agencies, service providers, schools, employers, and other organizations to support client goals.
- Network with local community resource providers to develop and maintain a referral and resource network.
- Attend relevant community resource and information-sharing groups to maintain knowledge of available services and strengthen community connections.
Information Management:
- Develop and maintain systems to track data and outcomes across programs.
- Maintain accurate records and log client interactions in databases.
- Write and deliver reports as required and/or requested.
KNOWLEDGE
- Knowledge of general record keeping practices and procedures.
- Knowledge of Oregon DHS Mandatory Reporting.
- Knowledge and application of stress management and wellness practices.
- Knowledge of current and relevant cultural and human diversity.
- Knowledge of the organization’s rules, regulations, procedures, Mission, Vision, and Values.

WORK ENVIRONMENT
Work is generally performed indoors, in various office or meeting room environments. This position is not normally exposed to hazardous materials, loud noises, or extreme heat or cold. Occasional exposure to chemical solutions, Airborne Pathogens, and Bloodborne Pathogens (body fluids) may occur. This position may require occasional overtime, evening, weekend, and/or holiday work. Work activities vary widely including attendance at meetings, trainings, and conferences; some overnight travel may be required. Local travel is required and requires the use of a personally owned vehicle, mileage reimbursement is available. Employees may use their personal cell phones for work-related purposes, such as business calls, emails, and accessing work-related applications. PHYSICAL AND MENTAL JOB REQUIREMENTS
To perform the job successfully, an individual must be able to perform each of the essential job functions satisfactorily. This position involves periods of prolonged sitting and standing and use of computer equipment. This position requires the physical and mental capabilities to read information printed on paper and displayed on computer monitors; hear, speak, and communicate verbally using the English language; cognitive thinking and mathematical calculation capabilities; manual dexterity to manipulate papers, files, keyboard, mouse, and telephone. This position requires walking, crouching, bending, stooping, twisting, turning, balancing, climbing steps/stairs, regular limb extension, pinching, grasping, moving, lifting, and carrying objects of 10 or more pounds frequently. MINIMUM EXPERIENCE AND QUALIFICATIONS
1. Bachelor’s Degree in Education, Social Work, Psychology, or a related field, OR an equivalent combination of education and experience.
2. Minimum of two (2) years’ experience working with at-risk clients.
3. Valid Oregon driver’s license at time of hire.
DESIRABLE EXPERIENCE AND QUALIFICATIONS
1. Master’s Degree in Psychology, Sociology, Human Services, or a related field.
2. Bilingual in English/Spanish
FLSA STATUS: Hourly, Non-Exempt
Salary Range: $19 - $21 per hour
