Position Physical Therapist Home Health Care Professional Location Richmond, Virginia Responsibilities Conduct assessments and develop customized treatment plans for clients based on their individual needs and goals. Provide skilled physical therapy services or home health care services to clients in their homes. Monitor progress, adjust treatment plans as needed, and communicate effectively with clients, families, and healthcare team members. Educate clients and their families on therapeutic exercises, home safety measures, and techniques for promoting independence and well‑being. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for clients. Requirements Valid license to practice physical therapy in the state of Virginia. Previous experience in home health care or physical therapy preferred. Strong clinical assessment, communication, and interpersonal skills. Compassionate and client‑centered approach to care. Age Friendly care experience. CHAPS accreditation knowledge or experience is a plus.
