Facility: GSS IA St Ansgar Ctr Location: Saint Ansgar, IA Address: 701 E 4th St, St Ansgar, IA 50472, USA Shift: Day Job Schedule: Full time Weekly Hours: 40.00 Starting Rate: $32.50 - $52.00 Job Details The Skilled Rehab and Home Health at Good Samaritan SE MN seeks to hire an Occupational Therapist to join our team. The position is full time, 35-40 hrs per week, M-F (weekdays/weekends/holidays can be flexible). Coverage includes the ComfortCare Skilled Nursing Facility in Austin MN and Good Samaritan Society St. Ansgar, Iowa, with assistance as needed in GSS Albert Lea and Home Health. Currently we treat orthopedic conditions, weakness, balance/falls, general deconditioning, and other various conditions. New grads welcome. Paid travel and mileage between facilities. Supervisor on site. Medbridge for Individual Exercise Programs and CEUs. Generous benefit package. Responsibilities Assesses clients/patients/residents and designs occupational therapy treatments. Helps clients/patients/residents develop, maintain and recover the ability to perform tasks in daily occupations. Conducts evaluations and provides interventions for functional, cognitive or behavioral impairments. Evaluates physical, cognitive, developmental and psychosocial abilities; develops realistic therapy goals and administers programs. Documents client/patient/resident information for evaluation; establishes therapeutic plans and modifies them as necessary. Provides education to clients/patients/residents to improve understanding and functional outcomes. Utilizes age-related treatments and evaluations relevant to clients/patients/residents. Demonstrates knowledge of current research in daily treatment. May treat individuals of all ages with varying diagnoses; documents according to departmental guidelines and regulatory requirements. Follows accepted clinical practice guidelines of the professional organization. Exhibits flexibility with variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Interacts with providers, case managers, family members, third party payers, public organizations, etc. as needed. May supervise assistants, aides, and students; provides ongoing education and training to peers as determined by leadership. Performs other related duties as requested by manager. Work focus may include post-acute, long-term care, and home health care environments depending on facility/location. Qualifications Appropriate education level required in accordance with state licensure. Must be licensed in the state(s) of practice as Occupational Therapist. When working at a Sanford Health Facility (not required but preferred for GSS Facilities): Must also be certified through the National Board of Certification in Occupational Therapy (NBCOT). May require valid driver’s license when applicable. Obtains and maintains required department-specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment. New graduates in the process of testing with the board may be considered with a temporary license in South Dakota and Iowa.
