Job Overview
Job Purpose:
- The Patient Navigator at Crescent Community Health Center is responsible for guiding patients through the healthcare system, ensuring they receive the necessary care and support. This role focuses on providing personalized assistance to patients, helping them overcome barriers to healthcare access, and improving their overall experience within the healthcare environment.
Key Responsibilities:
Assist patients in understanding their diagnosis, treatment options, and the healthcare process.
Coordinate appointments, follow-ups, and referrals to ensure timely access to healthcare services.
Provide emotional support and education to patients and their families, helping them make informed decisions about their care.
Identify and address barriers to healthcare access, such as transportation, financial constraints, and language differences.
Collaborate with healthcare providers, social workers, and community resources to develop comprehensive care plans for patients.
Maintain accurate and confidential patient records, documenting interactions and progress.
Advocate for patients' needs within the healthcare system to ensure they receive optimal care.
Conduct outreach activities to educate the community about available healthcare services and resources.
Participate in continuous training and professional development to stay updated on healthcare practices and patient navigation strategies.
Required Education:
- High School diploma or equivalent required. Bachelor's degree in Social Work, Public Health, Nursing, or a related field preferred.
Required Experience:
Experience in a healthcare setting, preferably in patient advocacy or care coordination.
Experience working with diverse populations and understanding of cultural competency.
Proven track record of successfully navigating patients through complex healthcare systems.
Required Skills and Abilities:
Strong interpersonal and communication skills, with the ability to build rapport and trust with patients and healthcare providers.
Excellent organizational skills and attention to detail, capable of managing multiple tasks and priorities efficiently.
Ability to work independently and collaboratively within a team environment.
Proficiency in using electronic health records (EHR) and other healthcare-related software.
Demonstrated problem-solving skills and the ability to think critically in fast-paced situations.
Knowledge of healthcare policies, procedures, and regulations, with a commitment to patient confidentiality and ethical standards.
