Social Services Director
Genesis HCC
Genesis HCC is seeking a Social Services Director to lead psychosocial care within our Healthcare & Medical Services department. In this on-site leadership role, you will oversee social services programming, conduct psychosocial assessments, and coordinate discharge planning to support safe, person-centered transitions. Collaborating closely with nursing, therapy, and medical teams, you will champion patient rights, regulatory compliance, and documentation standards while providing counseling and crisis intervention to residents and families.
