Job Description:\n\nDescription:\nSummary of Duties:\nThe House Lead is responsible for the one residence, the employees working within the residence, and the program participants residing in the residence who have intellectual, developmental and/or physical disabilities, assuring the facility operates in an efficient & effective manner. The House Lead is additionally responsible for scheduling, taking clients to appointments, being on call, coverings shifts, maintaining Therap, File Matrix, MAR, ItherapyDocs, teaching and training the staff in the residence you oversee. The House Lead oversees coordination and implementation of the Individual Service Plans (ISP), Behavioral Support Plan (BSP), Teaching Support & Strategies (TSS) & Healthcare/Crisis Plans for the program participants receiving services, provides behavioral support daily and in times of crisis, is part of the crisis team, supervises and trains employees at the residence, etc. The House Lead is an essential employee and responsible for the additional duties as listed below and/or assigned:\nPrimary Responsibilities/Essential Job Functions:\nCompany Policies & Procedures:\n· Signs & abides by the Employee Conduct Policy and all other policies and procedures.\n· Exemplifies company Mission.\n· Works schedule as assigned-maintaining good attendance and punctuality.\nMedication Supervision:\n· Ensures medications are ordered as required in a timely manner before the program participant runs out, stocked in the residential facility, picked up at the pharmacy or BLH office, if delivered, stored securely and appropriately in locked med boxes, then in locked cabinets/closet. i.e.: per individual in accordance w/AWMD rules.\n· Medical documentation reviewed, kept orderly and current, files/maintains medical documentation & other documentation.\n· Pharmacy book reviewed and kept orderly and current, logging medications in, files documents.\n· Ensures that medication labels coincide with the prescriptions and MARs.\n· Ensures the electronic MARs (medication administration records) are accurate and updated as needed.\n· Assists program participants with administration of medication & documents medication administration as required/needed.\n· Assures new or changed prescriptions get to the pharmacy within 24 hours and there is a current doctor order for the medication.\nIndividual Service Plans, Behavior Support Plans & Other Therapy Plans:\n· Ensures implementation and documentation of all ISP/IDT meetings & IST trainings.\n· Reads and reviews all progress notes, other monthly documentation (TSS, sleep logs, BM charting, MARs, task lists, etc.), timecards, personal vehicle mileage sheets, time of requests, etc. and all other electronic documentation completed by the employees you supervise.\n· Participates as a member of the interdisciplinary team for all ISP/IDT meetings.\n· Implements Individual Service Plans (ISP), Behavioral Support Plan (BSP) and any other therapy plans, trains all staff on those plans (new staff within 7 days, new plans within 14 days of receipt, annually with ISP) or coordinates trainings for those plans for their staff, documents training, assuring the staff are following the plans & filling out appropriate documentation for those plans.\nProgram Participant Needs:\n· Maintains primary on-call status for staff questions, staff coverage, non-emergency, and emergency situations for their facility.\n· Coordinates/maintains personal funds for individuals supported, keeping receipts for money the individual spends and turning in those receipts weekly/monthly to the BLH Administrative staff who oversees program participant finances unless noted otherwise in the ISP.\n· Coordinates transportation and staffing for program participants to and from therapies, work, day programs, community activities, appointments, emergency/crisis situations, etc.\n· Provides good customer service to program participants, stakeholders, team members, family members, guardians, etc.\n· Coordinates and takes program participants to all appointments (medical, employment, therapy, etc.), takes necessary forms to all meetings, appointments, and trainings to be signed or filled out by the appropriate doctor, therapist, etc., fills out all required documentation for those appointments, turns documentation (appointment forms, prescriptions, etc.) into the BLH office within a minimum of 24 hours. \n· Communicates information from medical/therapy appointments to team members including any medication changes as soon as possible and no later than 24 hours of the change. Team members included for every program participant are Case Manager, BSC, Guardian, Program Director, Lead Supervisor, Healthcare Coordinator, & RN(s). For notification, the house lead must send an e-mail to all the team members so there is a paper trail. You can notify by phone but in addition to that you must send an e-mail.\n· Ensure program participant’s personal needs are met.\n· Ensures menus are followed, and dietary, recreational, social, and personal hygiene needs are met. Trains employees in the facility on all these items. \n· Follows menus and prepares meals for program participants. Assures the staff in the residences are following menus and preparing meals for program participants.\n· Ensures the rights and confidentiality of program participants are protected.\n· Has access to all program participants’ files and information, may update/maintain computer databases with program participants’ information, maintains confidentiality in accordance with company policy and state, federal and HIPAA regulations.\n· Assures program participant’s medical and program records are reviewed, kept orderly and current, documents filed.\n· House Lead attends all program participant’s ISP/IDT meetings, trainings, any other meetings;\n· House Lead must inform RN(s) and Healthcare Coordinator by e-mail of all meetings, appointments, and trainings scheduled including dates and times & any rescheduled dates & times; \n· Attends House Lead/Supervisory meetings when assigned- informs staff of changes in policy or other necessary information discussed at the meetings.\n· Required to document in all company assigned electronic documentation modules including but not limited to Itherapy, Therap, Quickmar, HRIS/payroll/time keeping site, etc.\n· Maintains current First Aid and CPR certification.\nProgram Participant Personal and Social Activities:\n· Schedules, approves, and monitors outings and activities, and assures program participants attend regular scheduled activities such as Special Olympics, dance class, computer class, movies, bowling, etc., and that program participants arrive on time.\n· Coordinates cash needs to support outings for program participants and staff. \n· Support personal choice & meaningful day for program participants.\n· Attends and participates in Special Olympics events and other community events.\nMonitoring Staff:\n· Report suspected neglect, exploitation, or abuse of program participants.\n· Provides and coordinates safe and timely direct care service and/or customized community supports day services delivery.\n· Reviews, corrects, and approves time sheets, mileage sheets, progress notes, MARs, sleep logs, teaching support & strategies, task lists as well as other daily/monthly documentation, and assures they are completed by their due date. \n· Assures all Progress notes, sleep logs, teaching support strategies, MARs, task lists, incident reports, etc. are filled out by yourself and your staff daily when on shift.\n· Schedules and monitors all staff in their facility and monitors customized community supports day services staff in the community. Coordinates and documents work schedule changes, call-ins & training.\n· Participates in interviewing/hiring/termination process.\n· Participates in review/raise process.\n· Reports violation of job descriptions & policies, utilizing disciplinary action process, writing up employees as necessary.\n· Trains all newly hired/existing staff in their facility on the site orientation, incident reporting, employee time keeping, all required documentation, plans, on all paperwork, drills, policies, procedures in the residence, etc.\n· Schedules staff therapy trainings for program participants within 14 days of a plan coming out, records/documents the training and sends information to the office.\n· Required to review subordinate’s documentation in all company assigned electronic documentation modules including but not limited to Itherapy, Therap, Quickmar, HRIS, etc.\nMaintenance and Safety of House, Vehicles and Properties:\n· Monitors and schedules maintenance and repairs of the house, vehicles and equipment. \n· Reviews and initials vehicle log book/mileage sheets;\n· Reports neglect or damage to property.\n· Completes, monitors & implements cleanliness of house, including individuals’ rooms, and yard, ensuring household supplies are available at all times.\n· Reports any failure of functioning of the vehicle, fixtures, or other to the office immediately and discontinue use until the appropriate repairs have been made.\n· Practices and implements safe house keeping, reporting any safety hazards to the office immediately.\n· Reports any utility failures to the utility companies i.e.: gas, electric, etc. immediately (assure safety of the program participants first). If during office time, report to the office after assuring the health and safety of the individuals.\nAll Documents are sent to the BLH Office:\n· All documents including medication changes, doctor orders, doctor appointment forms, other appointment form, therapy reports, incident reports, ISP’s, BSP’s, etc. are turned into the BLH office within 24 hours.\n· Documents other needs, concerns, general progress and sends this information to the BLH office by e-mail or in writing.\n· Completes all monthly paperwork on time. \n· Completes disaster drills and safety trainings on time.\nFood Management:\n· Shop for and store healthy, nutritional food for the program participants. Assure there is an ample supply of food and menu items can be made daily.\n· Manage petty cash account for the residence, including program participants personal funds; \n· Monitor staff menu planning and reduce waste; Ex: leftovers used first, timely use of produce, etc. Assures that guidelines for food storage and disposal are followed.\n· Manages out to eat following approved restaurants and menu items to be purchased, assures staff are purchasing the same type of items as program participants. \nScheduling and On-Call:\nHouse Leads are required to complete schedules and make revisions in the time keeping system before payroll Mondays. House Leads are responsible for finding coverage for call-ins, leave for vacations/absences, etc. and must stay in contact with the Program Director/Lead Supervisor regarding coverage and keeping overtime at a minimum. House Leads are responsible for working the shift if coverage cannot be found. House Leads are responsible for attending Special Olympics and other company and/or program participant related events. Must be able to cover urgent care, crisis, and emergency situations.\nHouse Leads are required to have a cell phone at all times. Must be available to receive calls during shift, after hours, and on weekends for emergency, non-emergency situations, and staffing needs/questions (including staff coverage/crisis). House Leads must return emergency/crisis calls within 20 minutes, all other calls within 24 hours.\nCrisis Intervention Technique:\n· Follow all behavior support plans for program participant. \n· Utilize the Company’s approved crisis intervention technique if approved in the behavior support plan.\n· Employees must step back when a program participant has a challenging event and/or behavior. Restraint is used as a last resort. Utilize restraint only if the program participant is trying to physically hurt themselves, someone else or put themselves in immediate danger.\nMiscellaneous\n· Conducts monthly facility meetings.\n· Other duties as assigned.\n· Attends DOH DDSD required trainings, therapy trainings & facility, ISP/IDT & other meetings/community events as scheduled.\n· Must know and follow the current DD Waiver Service Standards.\nSafety Sensitive Position:\nThe House Lead position is a safety sensitive position where the employee holding this position has the responsibility for their own health and safety, as well as, the health and safety of the program participants, their employees, community members, team members and stakeholders. This position has many safety sensitive duties including but not limited to working directly with program participants, assisting with medication, preparing food, managing employees, and operating a motor vehicle regularly for the company.\nWorking Conditions/Physical Exertion/Dexterity:\n· Works primarily in the residential home and community. \n· May be requested to work other shifts or facilities; Shift times, days, position transfers, and facility assignments may change and occur as necessary and without notice.\n· Moderate physical exertion required, including but not limited to bending, sitting, standing, squatting, kneeling, running, walking, climbing stairs, participating in sport activities, etc. \n· Must be able to lift 25 pounds above the shoulders, 40 lbs. or more repeatedly, and support 70 to 100 pounds occasionally.\n· Must be able to move/respond quickly to many difficult situations and handle stressful situations calmly. \n· Ability to relate positively to the program participants and their teams. \n· Able to work directly with program participants including but not limited to challenging behaviors, physical restraint, medical care, personal care, grooming, hygiene, assisting with medication, lifting, and transferring. \n· Ability to work alone with program participants who are or can become verbally and/or physically aggressive daily and support them through crisis events. \n· Ability to hear and withstand repetitive noises, sounds, phrases, yelling, screaming, etc. \n· Ability to assist, support, lift and transfer individuals who utilize walkers, wheelchairs, and gait belts.\n· Able to perform the essential functions of the job with reasonable accommodation. \n· Able to work independently without constant supervision. \n· Ability to supervise staff, train staff & complete disciplinary process with staff.\n· Good verbal and written communication skills. Good organizational skills.
