As a Nurse Case Manager, you will play a central role in coordinating care, improving health outcomes, and supporting clients through proactive outreach and structured engagement. This is a highly collaborative role, working closely with Community Support Specialists and interdisciplinary care teams. What You’ll Do Collaborate with interdisciplinary teams to develop and implement client-centered care and wellness plans Provide structured outreach and engagement to maintain consistent client contact and support continuity of care Conduct health assessments, screenings (quarterly/annual), and wellness monitoring Support pediatric and adult clients with chronic medical conditions and behavioral health needs Monitor hospital admissions/discharges and complete medication reconciliation Coordinate care with external healthcare providers to ensure continuity and quality of services Deliver individualized health education and wellness support to clients and families Maintain accurate and timely documentation in alignment with program productivity expectations Track and support performance measures related to physical health outcomes How Success Is Measured This role includes structured expectations to support consistent client engagement and high-quality care delivery. Nurse Care Managers are expected to maintain regular outreach activity, complete timely documentation, and contribute to defined program productivity standards that are both achievable and supported by the team. Work Environment & Travel Up to 25% local travel for client engagement in the community or home settings Travel is purposeful and non-routine based on client need Nurses are not expected to transport clients as a standard responsibility When transportation support is required, it is coordinated alongside a Community Support Specialist and not performed independently Not expected to attend routine client appointments in external healthcare settings Requirements Of The Position Graduation from an accredited nursing program (LPN or RN) Active, unrestricted Missouri nursing license Valid driver’s license, proof of insurance, and reliable transportation Pediatric or adolescent experience strongly preferred (experience working with children ages 5+) Experience working in mental health, behavioral health, or substance use settings preferred (2+ years ideal) Strong communication, engagement, and client-centered care skills Comfort working in electronic health records and standard Microsoft Office tools Ability to work both independently and as part of a highly collaborative team Grow With Us At ReDiscover, we invest in your growth as a Nurse Care Manager through ongoing training, interdisciplinary collaboration, and meaningful work that directly impacts the lives of individuals and families in our community. #J-18808-Ljbffr
