Registered Nurse - Hospice

Job Description

Job Summary:
Provides comprehensive hospice nursing care to patients in their homes, ensuring smooth coordination of interdisciplinary services and adherence to regulatory standards. Focuses on patient assessments, education, care planning, and medication management to improve quality of life.

Responsibilities:

• Assess patients' appropriateness for hospice care, learning needs, and progress toward goals.
• Develop and modify individualized care plans based on evidence-based practices and patient needs.
• Ensure timely completion of admission, re-visit assessments, and discharge assessments, including data transfers.
• Educate patients and families using age-appropriate methods, evaluating and revising teaching plans as necessary.
• Supervise Home Health Aides, ensuring quality care delivery and compliance with standards.
• Communicate effectively with the interdisciplinary team, participating in bi-weekly meetings to coordinate patient care and progress.

Qualifications:

• Valid New Jersey RN license with current BLS certification.
• Minimum of 2 years of home hospice RN experience required; 3+ years preferred.
• Proven proficiency with EMR systems and excellent critical thinking and communication skills.
• Valid New Jersey driver's license and reliable transportation.
• Knowledge of hospice regulations, insurance reimbursement, and safe medication management.
