Hospice RN

Schedule

This position offers a shift from 4 PM to 8 AM, with a commitment of three shifts per week. The role requires on-call coverage for Morris and Sussex Counties, along with a weekend requirement every three weeks, for which compensation days will be provided. One summer and one winter holiday are included.

Job Description

The Hospice RN is responsible for performing patient assessments and developing individualized plans of care based on patient and family needs. This role emphasizes the proper utilization of hospice services while ensuring compliance with care standards and regulations.

Responsibilities

• Conduct accurate patient assessments to determine appropriateness for hospice care.
• Develop and implement effective care plans in collaboration with interdisciplinary teams.
• Maintain productivity standards and ensure regulatory compliance.
• Document all patient interactions, assessments, and care plans using electronic health records.
• Participate in interdisciplinary team meetings to evaluate and revise care plans as necessary.
• Mentor and provide orientation to new staff members.
• Engage in performance improvement activities to enhance care quality.
• Ensure a safe environment in accordance with infection control protocols.

Required Experience / Certifications / Licensure

• Current RN license in the state of practice.
• Experience in hospice or palliative care preferred.
• Knowledge of insurance reimbursement and hospice regulations.
