Current California State NP or PA license

\n

Current Nurse Practitioner Furnishing Number (for NPs)

Nurse Practitioner furnishing number of NPs (for NPs)

CAPA Controlled Substance Certification (for PAs)

\n

Active DEA registration

\n

\n

Preferred:

6 months or more of related clinical experience in primary care
