Medicare Outreach & Community Growth Specialist
Hispanic Alliance for Career Enhancement
Oak Street Health is seeking an Outreach Executive I to build relationships with Medicare seniors and convert them into patients within the local community. You will work closely with Outreach Managers and Center teams to drive new patient growth through events, referrals, and direct outreach. Role emphasizes proactive lead generation, CRM updates, and collaboration with a cross-functional team.
