As an Afterhours RN Care Manager at Gentiva, you will provide home-based, afterhours clinical triage, nursing interventions, and care coordination for patients and families. You’ll assess patient needs by phone and in the home, manage urgent symptoms, implement care plans, and collaborate with on-call providers and the interdisciplinary team. In Gentiva’s compassionate, mission-driven culture, you’ll support patient dignity, safety, and comfort while documenting care, educating caregivers, and ensuring seamless handoffs to the daytime care team.
