Title
Transplant Navigator / Case Manager - Hematology/Oncology

Schedule
Monday-Friday, 08:00-16:30

Assignment Details
Contract Length: 12 weeks
Guaranteed Hours: 40 hours/week

Requirements
- Minimum 3 years clinical experience required
- Hematology/Oncology experience required
- Navigator/Case Manager experience required
- Required setting experience: Acute hospital and outpatient clinic
- Required clinical focus: Pre-transplant evaluation/assessment and transplant coordination
- BLS certification required
- Epic charting system experience required
- Preferred: Chemo certification (ONS/ONCC)

Roles & Responsibilities
- Conduct pre-transplant evaluations and patient assessments.
- Coordinate all aspects of patient care before, during, and after transplant.
- Schedule and track pre- and post-transplant testing and appointments.
- Monitor patient vitals, fluid balance, and graft function; identify and manage complications.
- Educate patients and families on transplant procedures and medication adherence.
- Collaborate with the transplant team to ensure adherence to center protocols.

Compliance / Notes
- Orientation: 40 hours (basic) with ongoing training throughout the assignment.
- Pre-hire testing must be completed within 48 hours after signing.
- Flu vaccine required (medical/religious exemptions only).
- Interview required prior to offer; candidate availability should be provided.
