Travel Hospice Registered Nurse (RN) – Case Manager ($2,486/Week)

Location: Grand Junction, CO
Start Date: July 13, 2026 (Quick Submission Recommended)
Contract Length: 13 Weeks
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Guaranteed Hours: 40 Hours/Week

Roles & Responsibilities

• Manage a caseload of hospice patients in community settings.
• Develop and coordinate individualized care plans with the interdisciplinary team.
• Perform comprehensive patient assessments and manage symptoms effectively.
• Educate patients, families, and caregivers on hospice care and disease management.
• Deliver compassionate end-of-life care while maintaining accurate clinical documentation.

Bonus & Benefits + Salary

• Weekly Gross Pay: $2,486
• Tax-Free Weekly Stipend: $1,246
• W-2 Pay: $31/hour
• Overtime: $77/hour after 40 hours
• On-Call Pay: $2/hour (Pager)

Mandatory Requirements to Apply

• Active Colorado RN License (or eligible to practice in Colorado)
• Minimum 2 years of Hospice RN experience
• Previous Case Management experience preferred
• Strong patient assessment and symptom management skills
• Ability to travel to patient homes, nursing homes, assisted living, and group homes
• Excellent communication and interdisciplinary teamwork skills
• Holiday coverage required
