Care Navigator (Medical Assistant or LPN)

Job Overview Position: Chronic Care Management Specialist (Care Navigator). Location: Morganton, NC – onsite in an outpatient care clinic. Schedule: Monday‑Friday, with occasional weekend work to close out month‑end processes. Responsibilities Be a health hero to over 500 patients across North Carolina. Build lasting relationships through regular check‑ins and support calls. Use state‑of‑the‑art remote monitoring tools to track patients' health in real‑time. Collaborate with clinical pharmacists to optimize patient care plans. Educate and motivate patients to achieve their health goals. Be the friendly voice and listening ear that patients look forward to hearing from. Help reduce hospital readmissions, improve medication adherence, and enhance patients' quality of life. Empower patients to take control of their health and provide peace of mind to patients and families. Qualifications Certified Medical Assistant (MA) with a passion for patient care. Empathetic individual with excellent communication skills. Tech‑savvy professional comfortable with digital health platforms. Detail‑oriented team player who can juggle multiple priorities. Eager to learn and grow in an innovative environment. Required qualifications (one of the following): Nationally recognized MA certification or an LPN license.
