Cadre Hospice LLC is recruiting an RN Case Manager who will be the primary clinical advocate for patients, coordinating personalized end-of-life care aligned with each patient’s goals, wishes, and needs. You will collaborate with physicians, social workers, chaplains, aides, volunteers, and other team members to ensure compassionate care, educate patients and families, manage symptoms, and support families through difficult conversations and care planning. #J-18808-Ljbffr
