Medical Social WorkerAll KHI employees are expected to perform their respective tasks and duties in such a way that supports KHIs vision to be among the kindest, highest quality health systems in the country. Under the direction of the designated physician and clinical care providers, work collaboratively with multidisciplinary team members, patients, families, and community agencies to identify appropriate community resources to achieve desired patient outcomes.Key ResponsibilitiesStatus and Level of Care ManagementCollaborates with the interdisciplinary team members (including physicians, providers, licensed mental health practitioners, case managers, nurses, and other members of the care team) to ensure comprehensive care for at-risk patients identified by the case manager.Provides education/consultation to the Nursing Staff to ensure achievement of quality patient outcomes and patient satisfaction.Educates patients regarding regulatory and insurance processes and requirements (i.e. AHCCCS and Medicare).Maintains accurate and timely records, including clinical and progress notes, according to designated policy and procedure.Provides social services by utilizing knowledge and skills necessary to effectively meet the growth, development and continued clinical needs for patients of all ages from newborn through geriatric.Proactively acts as a liaison between the patient, family, medical staff, nursing staff, ancillary personnel and agencies involved in the patient's hospitalization and continuing care.
