Registered Nurse - Weekend On Call

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Weekend On Call team at Harmony Hospice in Chicopee, Massachusetts. As a valuable member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and their families, focusing on comfort, dignity, and support. Responsibilities:

Provide weekend on-call coverage for our patients and families, responding to emergencies and supporting the care team as needed
Conduct assessments, develop, and implement individualized care plans, and coordinate care with other healthcare professionals
Manage pain and symptoms, providing medication and other therapeutic interventions as needed
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and coordinated support
Participate in multidisciplinary team meetings to develop and refine patient care plans
Maintain accurate and detailed records of patient interactions, assessments, and care provided
